Sometimes we hit the following error during a deployment:
Aws::AutoScaling::Errors::Throttling: Rate exceeded (Aws::AutoScaling::Errors::Throttling)

The instance refresh continues in the background but the capistrano task fails.

We should probably gracefully recover from this and back off and retry until we pick up responses again.
