EPIC: VS Code extension (v0.8.8 Phase 1 tracker)

Summary

Umbrella for the VS Code extension effort. Phase 1 is an attached chat panel backed by the existing app-server HTTP/SSE surface. Phase 2 cursor-like depth stays deferred unless reprioritized.

Acceptance criteria

• Track the Phase 1 child issues below.
• Keep implementation local-first and compatible with Cursor/VSCodium via OpenVSX.
• Do not treat ACP as required for the first webview milestone, but keep it visible as architecture leverage.
